Regulatory & Oversight Bodies

1. College of Physicians and Surgeons of Ontario (CPSO)

• Provincial regulator responsible for licensing physicians in Ontario

• Issues certificates of registration (independent, supervised, restricted)

• Oversees professional conduct, quality assurance, and discipline

2. Medical Council of Canada (MCC)

• National body that assesses medical credentials and exams

• Administers:

  • MCCQE Part I

  • NAC Examination

Overview of IMG Licensing Pathways in Ontario

There is no single pathway for all IMGs. The route depends on:

• Country of medical education

• Years and type of postgraduate training

• Specialty vs. family medicine

• Exam history

STEP‑BY‑STEP PROCESS FOR IMGs

STEP 1 - Create an MCC Account & Verify Credentials

All IMGs must start with the Medical Council of Canada (MCC).

Actions:

• Create an account on physiciansapply.ca

• Submit:

  • Medical degree diploma

  • Transcripts

  • Internship certificates

• Documents are source‑verified by MCC

Timeline: 2 - 6 months (can be longer)

Cost: Several hundred CAD (varies by documents)

Credential verification is mandatory for all pathways.

STEP 2 - Complete Required Examinations

1. MCCQE

• Computer‑based exam (medical knowledge + clinical decision‑making)

• Required for:

  • Most CPSO registration routes

  • Residency (CaRMS)

2. NAC Examination (OSCE‑style)

• Clinical skills exam using standardized patients

• Required for:

  • CaRMS (IMG residency stream)

  • Practice‑Ready Ontario (PRO) pathway

STEP 3 - Choose a Licensure Pathway

PATHWAY A - Residency Training in Canada (CaRMS IMG Stream)

This is the most common route for IMGs without recognized postgraduate training.

Requirements:

• MCCQE Part I

• NAC Exam

• Verified credentials (MCC)

• English or French proficiency

Process:

• Apply through CaRMS (IMG stream)

• Interview with Ontario medical schools

• Match to a residency position (Family Medicine or Specialty)

Ottawa note:

• Residency positions may be at University of Ottawa and affiliated hospitals (TOH, CHEO, Montfort).

Timeline:

• 2-5 years (includes residency training)

Highly competitive - limited seats for IMGs

PATHWAY B - Practice‑Ready Ontario (PRO)

For experienced physicians who have:

• Completed postgraduate training abroad

• Practised independently in another country

Requirements:

• MCCQE Part I

• NAC Exam

• Recent, independent clinical practice

• Specialty or Family Medicine alignment

Process:

• Clinical assessment (supervised practice)

• If successful → CPSO certificate with supervision

Often focused on underserviced communities, but some placements may be within or near Ottawa.

Timeline:

• 6-18 months (assessment + registration)

  
  

PATHWAY C - Specialty Recognition (RCPSC / CFPC)

For physicians already trained as specialists or family physicians.

Two possible outcomes:

• Substantial equivalence → eligible for certification exams

• Partial equivalence → additional training required

Timeline:

• 1–3 years depending on outcome

STEP 4 - Apply to CPSO for Registration

Once exams and training requirements are met, apply to CPSO.

Types of Certificates:

• Independent Practice

• Supervised Practice

• Restricted / Academic

CPSO Requirements:

• MCC credentials verified

• Certification (CFPC or RCPSC)

• Proof of good standing

• Language proficiency

STEP 5 - Begin Practice in Ontario

Once licensed:

• Secure hospital privileges (if applicable)

• Register with OHIP

• Arrange CMPA coverage
